The City of Texarkana, TX - Vital Statistics does not accept credit cards for online orders; however, for your convenience, you can process online requests through an independent company that we have partnered with to provide you this service-VitalChek Network, Inc.

Regular mail is processed within 14 business days upon receipt.
Provide the following for mail-in request processing:

- Substitute Application for Notarized Statement (PDF)
- Make a copy of the qualified applicants identification
- Secondary ID Sheet (PDF)
- Include payment, check, or money order / cashier's check

Amendment of a Birth and / or Death Certificate
An amended certificate may be filed to complete or correct a record that is incomplete or proved by satisfactory evidence to be inaccurate. The amendment must be in a form proscribed by the department. The amendment shall be attached to and become a part of the legal record of the birth, death, or fetal death if the amendment is accepted for filing.

The fee to file an amendment is $15.
Each requested certified copy of the updated birth record is $22.
1st requested certified copy of the updated death record is $21.
Additional certified copies of the updated death record are $3.

A qualified applicant is defined as the registrant, immediate family member, or legal representative. Local, state, and federal law enforcement, governmental agencies and other persons may be designated as properly qualified applicants by demonstrating a direct and tangible interest in the record when information in the record is necessary to implement a statutory provision or to protect a personal legal property right. A properly qualified applicant may also be any person who has submitted an application for a request to release personal information and has been approved as outlined in the Health and Safety Code, Section 181.11, relating to Requests for Personal Data.
In Texas, Outside of Texarkana, Wake Village, or Nash
The City of Texarkana, TX issues Birth Abstract certificates for births in Texas, but outside of the cities of Texarkana, Wake Village, and Nash. These documents are issued via remote access directly from the State of Texas, Vital Statistics Unit in Austin, Texas. The Birth Abstract certificates are legal documents and can be used for the same legal purposes as certificates generated from Texarkana files for institutional or hospital births. The Passport Agency and requests for obtaining Indian Role Numbers may request the Long Form Birth Certificate which is only obtainable through the city or county the birth occurred in, or through the State of Texas Vital Statistics Office.
Death certificates for deaths occurring outside Texarkana, Wake Village, or Nash, TX are available in the county of which the death occurred, or from the State of Texas Vital Statistics Office.
Death records older than 25 years and birth records older than 75 years are, with some exceptions, available to genealogists. Each document search must be paid for and sufficient information must be provided on the application for the document search. The Texas State Library has research material available specifically for genealogy research.
